Please print the Patient Registration, Medical History and HIPAA Consent forms and complete them prior to your visit. Please review our Notice of Privacy Practices before signing the HIPAA Consent Form.
We are committed to protecting the safety of our patients, staff, and the general public while maintaining high-quality care at our clinic and would like to update you on recent steps that we have implemented in response to the Coronavirus (COVID-19) outbreak.
We will begin seeing patients for scheduled visits starting on May 4, 2020. Please feel free to call us and leave a message (719)421-7132 or email us at firstname.lastname@example.org for scheduling.
We believe the following office protocols will keep you safer:
We will ask screening questions prior to your scheduled appointment and if you or anyone in your family meets certain criteria, we will reschedule your appointment to a later date.
We have a visit restriction in place. To protect you, other patients, and their families, no significant others, children, other family, or friends will be permitted in the office during your visit, unless necessary (e.g., a support person or supported decision-maker for patients with disabilities who require assistance).
We request you to wear a cloth mask during your visit, as recommended by the CDC.
When you arrive at our office while still in your car, please call us at 719-421-7132 and alert us that you have arrived. We will then tell you whether we are ready to see you or call you back shortly, as we want to make sure we have a sterilized exam/procedure room available for you immediately upon entry. We are limiting waiting room exposure between patients at this time.
Upon entering our suite, hand sanitizer will be available.
As a reminder, the best prevention you can take is to follow the [Stay at home/Shelter at home] order currently in place. If you must go out, remember to practice social distancing and frequently wash your hands or use alcohol-based hand sanitizer. Please contact your primary care physician or the El Paso County Department of Public Health if you are experiencing symptoms and think you need to be tested. We thank you for your understanding if we may need to reschedule your appointment in accordance with the recommendations set forth by different agencies.
